Assumption of Risk & Release
In consideration of being permitted to participate in any Huddle Action / One Huddle One Mission event, activity, or program (the "Event"), including but not limited to bounce houses, inflatables, bumper cars, youth games and activities, and food vendor services, I, the undersigned (individually and/or as parent/legal guardian of the minor participant(s) listed below), acknowledge and agree to the following on behalf of every participant registered on this waiver:
- Assumption of Risk. I understand that participation in the Event involves inherent risks including, without limitation, physical injury, collisions, falls, allergic reactions, food-borne illness, exposure to weather, contact with other participants, and property damage. I knowingly and voluntarily assume all such risks for every participant listed on this form.
- Release & Waiver. I hereby release, waive, discharge, and covenant not to sue Huddle Action, One Huddle One Mission, their organizers, directors, officers, employees, volunteers, sponsors, vendors (including food and inflatable operators), venue owners, and affiliates (collectively, the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained during or in connection with the Event.
- Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs (including attorneys' fees) they may incur due to any listed participant's involvement.
- Medical Consent. I authorize the Released Parties to secure emergency medical treatment on behalf of any listed participant if necessary, and I am responsible for all related costs.
- Media Release. I grant permission for photos and video taken at the Event to be used by Huddle Action and One Huddle One Mission for promotional and community purposes without compensation.
- Rules & Conduct. Every listed participant agrees to follow all posted rules, staff and volunteer instructions, and vendor guidelines, and to behave in a safe and respectful manner.
- Minors. If any participant is under 18, I represent that I am the parent or legal guardian with authority to sign this release on their behalf, and I bind each such minor to all its terms.
- Severability & Governing Law. If any provision is held unenforceable, the remainder shall remain in full force. This agreement is governed by the laws of the state in which the Event is held.
I have read this waiver, understand it is a release of liability and contract, and sign it voluntarily on behalf of every participant registered below.
